If you have a valid Health & Welfare Lasting Power of Attorney (LPA), registered with the Office of the Public Guardian, please provide a copy of the document along with proof of identity for the attorney(s) to the practice (either in person or via email). Please note that GP practices do not create or complete LPAs; these must be arranged directly through the Office of the Public Guardian or a legal professional.
Once verified, we will record the LPA on the patient’s medical record so that our team are aware of who is authorised to be involved in care and communication. In line with the Mental Capacity Act 2005, where a patient lacks capacity to make a specific healthcare decision, we will work with the appointed attorney to support decision-making and ensure appropriate communication.
